CARDIOLOGY

Renal Biospy

Renal (Kidney) biopsy is a diagnostic procedure in which a small piece of renal tissue (tissue from the kidneys) is obtained to know the cause of renal failure, to know the cause of Proteinuria and to know the severity in some renal diseases like Lupus Nephritis.

An ultrasound-guided renal biopsy can be that of the native kidneys or that of the kidney transplant is routinely performed under local anesthesia in adults but children need general anesthesia. Bedside renal biopsies are performed for sick patients in the ICU and in the ward.

The patient needs to get admitted on the morning of the procedure and need to be under complete bed rest for a day after the procedure. He can resume his normal activities a couple of days after the procedure.

Some investigations like coagulation parameters need to be checked prior to renal biopsy. Complications like blood in the urine can occur in about 5% of patients. Rarely the bleeding can be severe to warrant another procedure to stop the bleeding.
Hemodialysis
Haemodialysis is performed at SPT Hospitals with sophisticated equipment (volumetric, bicarbonate machines). We have 14 state-of-the-art machines [13 of them in the haemodialysis unit and 1 in the intensive care unit (ICU)].
A state-of-the-art reverse osmosis plant purifies the water used for haemodialysis. Periodic cultures of the reverse osmosis water are performed to ensure good quality of water.

Renal Transplant
A kidney transplant is a surgical procedure to place a healthy kidney from a living or deceased donor into a patient having irreversible advanced kidney disease (end-stage renal disease). Kidney transplant is considered to be the best treatment option as it can increase one’s life expectancy.
Kidney transplantation is classified into deceased donor transplantation or living donor transplantation depending on the source of the donor organ.
Kidney (renal) transplantation is a surgical procedure in which a healthy kidney from a person (donor) is removed and placed in a patient (recipient) with irreversible advanced kidney disease (end-stage renal disease; ESRD).
Extracarporial Theraphy
Plasmapheresis is the procedure which is done to exchange the plasma present in the blood with fresh plasma substitute or plasma from a donor. Plasmapheresis or plasma exchange process broadly involves the following steps:

Blood is withdrawn from the patient’s body to the apheresis machine through one of the catheter tubes. After the blood enters the machine, the blood cells will be segregated from the plasma.
Renal Stone

Initial treatment will focus upon the relief of pain. After this, the next step will be to facilitate the passage of the stone or the removal of the stone itself.
80-90% of all stones smaller than 5 mm will pass out on their own. If the stone is smooth, even stones of 7-8 mm may pass out on their own.

Stones larger than this will invariably need to be removed by one of the many methods available. If there is an anatomical abnormality, the priority will be to correct that abnormality while removing the stone.

1-2 cm solitary stones in the kidney is preferably treated by ESWL. This is a method by which the stone is powdered when pressure waves from a machine are focused on the stone. The stone fragments will then pass out in the urine stream over a period of 1 to 12 weeks.
Larger stones in the kidney are preferably removed by PCNL. In this method, the patient needs to be admitted to the hospital. A small puncture is made from the back directly into the kidney, the stone is identified, fragmented and completely removed.
